AUTHORIZATION TO RELEASE SCHOOL RECORDS

In accordance with the “Family Educational Rights and Privacy Act” you must request your previous school to release school record information. 

So that you may start as soon as possible, print a copy of the release form and fill it out.  Fax it to us at 618-465-5489 and send it to your previous school.
